What is Ultherapy Prime?

Ultherapy Prime is the upgraded version of the Ultherapy platform.

It combines micro-focused ultrasound energy with real-time ultrasound imaging, allowing the treating doctor to visualise the tissue beneath the skin before and during treatment.

Focused ultrasound creates precise points of controlled thermal injury within collagen-rich tissue. This initiates the body’s natural healing response, stimulating new collagen formation and remodelling over the following months.

How does Ultherapy Prime work?

One of the advantages of Ultherapy is that we can treat the face at different depths, depending on what we are trying to achieve.

At 1.5 mm, treatment is focused within the more superficial dermis. Here, collagen remodelling can help improve skin firmness and quality.

At 3.0 mm, treatment reaches the deeper dermis and superficial supporting tissues. Depending on the anatomy of the area, this may include parts of the fibrous connective-tissue network that links the skin to the deeper facial structures, helping to improve tissue support.

At 4.5 mm, treatment reaches deeper supporting tissue, which may include the SMAS. The SMAS — or superficial musculoaponeurotic system — is part of the deeper structural support of the face and is also a layer surgeons work with during surgical facial lifting.

By treating at different depths, Ultherapy can therefore address both the quality of the supporting tissue and deeper facial support, with the treatment pattern tailored to what each area of the face needs.

Ultherapy does not reproduce a surgical facelift. It does not remove excess skin or surgically reposition facial tissues. Instead, focused ultrasound creates precise points of controlled heating within the selected tissue layers, stimulating contraction followed by new collagen formation and remodelling over the months that follow. The result is a gradual improvement in facial support, definition and contour.

Importantly, facial anatomy varies from person to person — and even between different areas of the same face. A 4.5 mm treatment depth does not automatically correspond to the SMAS everywhere.

This is where real-time ultrasound visualisation becomes particularly valuable. Your doctor can see the tissue beneath the applicator and select the appropriate treatment depth according to your individual anatomy.

Ultherapy Prime vs Original Ultherapy

Ultherapy Prime builds on the same established micro-focused ultrasound technology with real-time visualisation as the original Ultherapy.

The Prime platform offers improved ultrasound visualisation with a wider field of view, allowing your doctor to see the underlying tissue more clearly when planning and delivering treatment. Treatment is also more efficient, with many patients reporting an improved treatment experience and greater comfort.

Visualisation: Retains DeepSEE imaging, with a larger display, an expanded ultrasound image field, and changes to image processing. These changes affect how the doctor views and works with the tissue image.
Treatment speed: Session length depends on the areas treated, line count, imaging and time taken to adjust the treatment plan. Treatment speed: Uses an updated operating platform. Merz reports an average 20% increase in system speed from a provider survey; this does not mean every appointment is 20% shorter.
Comfort: Energy pulses can cause warmth, tingling and discomfort. The treatment area, energy settings and individual sensitivity influence what you feel. Comfort: Sensations still vary between individuals. Changes to the platform do not make treatment painless, and your doctor should discuss pain management before starting.
Transducer design: Uses interchangeable imaging and treatment transducers with focal depths of 1.5, 3.0 and 4.5 mm. What areas can Ultherapy Prime treat?

Ultherapy Prime is particularly suited to patients beginning to notice softening or descent of their facial contours rather than simply changes at the surface of the skin.

Brow

As the tissues around the upper face change with age, the brow can gradually descend, and the eyes may begin to appear heavier.

Ultherapy can be used to produce a subtle brow lift in appropriately selected patients, creating a more open appearance around the eyes.

Loss of definition along the jawline and early jowling are common reasons patients seek treatment.

By treating the appropriate supporting tissue, Ultherapy can help create a more defined, lifted lower-face contour.

Ultherapy can help improve softening and loss of definition beneath the chin when tissue descent is contributing to the change.

If the main concern is excess fat rather than tissue descent, another treatment may be more appropriate. Distinguishing between the two is an important part of your assessment.

Ultherapy can be used to improve early tissue descent and loss of definition along the neck, with treatment depth and distribution adapted to the anatomy of the area.

Ultherapy is also FDA-cleared to improve lines and wrinkles of the décolletage.

Who is a good candidate for Ultherapy Prime?

Ultherapy tends to be most appropriate when there is early to moderate tissue descent and enough existing tissue support to respond to a non-surgical lifting treatment.

You may be suitable if you are beginning to notice:

Age alone does not determine whether Ultherapy is suitable.

Someone in their 30s may have anatomy that makes them an appropriate candidate, while another person at the same age may have very little tissue descent and gain limited benefit from treatment. Similarly, more advanced facial descent or significant excess skin may be better addressed surgically.

The most important question is therefore not simply “Am I old enough for Ultherapy?” but “Is the change I am seeing something Ultherapy can realistically improve?”

What happens during an Ultherapy Prime treatment?

1. Facial assessment

Your treatment begins with an assessment of your face as a whole.

We look at where facial tissues have changed, the pattern of tissue descent, facial proportions, skin quality, and whether other factors — such as volume loss or skin laxity — are contributing to what you see.

This distinction matters. Not every poorly defined jawline requires lifting, and not every flattened cheek requires filler.

After cleansing and marking the treatment areas, ultrasound gel is applied.

Your doctor uses the Ultherapy Prime imaging system to visualise the tissue beneath the skin and identify the appropriate layers for treatment.

The treatment pattern, depths, and number of lines are then planned according to your anatomy.

Ultrasound energy is delivered along the mapped areas.

During each pulse, you may experience warmth, tingling or a deeper aching sensation. Comfort varies between patients and between different parts of the face.

Treatment typically takes approximately 45 to 90 minutes, depending on the areas being treated and the complexity of the treatment plan.

There is usually minimal downtime.

Some patients experience temporary redness, swelling, tenderness, or altered sensation after treatment. These effects generally settle progressively.

Most patients can return to their usual daily activities soon afterwards.

When will I see results?

Ultherapy is designed to produce a gradual rather than immediate transformation.

Some patients begin to notice an early change within the first month. More visible improvement typically develops over approximately two to three months, as collagen production and remodelling progress.

The face may gradually appear:

  • Better defined through the jawline
  • Less heavy through the lower face

How long do Ultherapy results last?

Results can remain visible for approximately a year or longer, although longevity varies considerably between individuals.

Ultherapy does not stop the ageing process. Your underlying anatomy, skin quality, degree of tissue descent, age, weight changes, and individual collagen response all influence how your result evolves.

Rather than prescribing treatment automatically at a fixed interval, we prefer to reassess the face and determine whether — and where — further treatment would be beneficial.

A “line” refers to a sequence of focused ultrasound treatment points delivered beneath the skin. While line count contributes to treatment cost, more lines do not automatically mean a better treatment.
